Required for all equipment rentals
This is a legally binding document. Please read it carefully before signing. This form must be completed by a responsible adult (18 years or older) who will be present for the duration of the equipment hire. Your completion of this form confirms your understanding and acceptance of all terms outlined herein, in compliance with the laws of the Republic of South Africa.
This Indemnity and Liability Waiver Agreement (the "Agreement") is entered into by and between Mini Playtopia (the "Supplier") and the undersigned individual (the "Client" or "You"), who is hiring equipment for a private event.
You acknowledge that the use of soft play equipment, bounce houses, inflatables, and any other items provided by Mini Playtopia (the "Equipment") involves inherent risks. These risks include, but are not limited to, the risk of physical injury (including minor, serious, and catastrophic injuries), emotional distress, and damage to property, which may arise from your own actions or inactions, the actions or inactions of other participants (including minors), or the condition of the Equipment and the event premises.
You understand that while Mini Playtopia takes reasonable steps to provide safe equipment, we cannot eliminate all risks. You voluntarily assume full responsibility for all risks involved in the use of the Equipment by yourself and any and all participants at your event, particularly minors under your supervision.
In consideration for being permitted to hire and use the Equipment, and in full recognition of the inherent risks, you hereby agree to the following:
You confirm that you have received, read, and understood the full safety rules provided by Mini Playtopia. You agree to enforce these rules at all times during the event and to ensure constant, responsible adult supervision of any minors using the Equipment. You accept full responsibility for the safety and conduct of all participants.
By signing this document, you confirm that you are at least 18 years of age, have read this Agreement in its entirety, fully understand its terms, and are signing it freely and voluntarily. You understand that you are waiving certain legal rights that you or your heirs, next of kin, executors, and administrators may have against Mini Playtopia.
